Care Connect is a Not for Profit organisation specialising in assessment, case management and brokerage services. With funding provided by both State and Commonwealth Governments, Care Connect operates in Victoria, New South Wales and Queensland.
Formerly known as West Care Linkages, the organisation was established in Melbourne in October 1994, specifically to prevent the premature and inappropriate admission of frail aged and younger disabled persons to residential care (ie: nursing home and hostels).
Challenge
A multi-state community-care organisation working across assessment, case management and brokerage operates inside funding regimes that have changed substantially over the life of the organisation. Maintaining strategic clarity and a coherent service model across jurisdictions, while navigating successive phases of aged care and community-care reform, is the standing challenge for the board and senior team.
Approach
Australian Strategic Services worked with Care Connect’s leadership team on governance and strategic work appropriate to a multi-state community-care provider. The engagement focused on the strategic priorities the organisation needed to act on, the governance arrangements suited to its scale and footprint, and the leadership disciplines that connect strategic intent with operational delivery.

The work supported the board and senior team in clarifying strategic direction and governance practice through a period of significant change in the community-care funding environment.
